Let's talk a little pituitary anatomy physiology.

in the middle of the anterior pituitary gland.

Now, the anterior pituitary gland secretes a

lot of stuff. It secretes growth hormone,

thyroid stimulating hormone (TSH), follicle stimulating

hormone (FSH), luteinizing hormone (LH),

adrenocorticotrophic stimulating hormone or ACTH

and melanin stimulating hormone (MSH).

So that's the anterior gland.

In the posterior gland, you're going to have secretion of

antidiuretic hormone and oxytocin which control breast

milk secretion, hyponatremia or serum sodium, and other

factors that are a little bit beyond

the scope of this discussion now.

But let's look at the anatomic position of structures in

the pars distalis or anterior portion of the

pituitary gland known as the pars distalis.

Here's the tuft. And right next to it,

these little dark areas known as the mucoid ridge.

Found here are ACTH and TSH secreting microadenomas.

Now, this is important because surgical indication for

microadenomas is much stronger when you have ACTH

secretors or in the lateral wedge where you find growth

hormone secretors that produce either gigantism in the

adult or acromegaly in the younger individual.

What else is found out here in the lateral wedge?

The common prolactin-secreting microadenomas.

So prolactin and GH secretors out here,

ACTH and TSH secretors in here in the mucoid wedge.

And then finally, FSH and LH secretors,

quite uncommon,

can occur anywhere in the gland because

they're scattered throughout the gland.
