Differentiating Between Type 1 & 2 Lunates

Dr. P and Dr. Stern here talking about a 21-year-old

0:01

3 00:00:01,630 --> 00:00:03,530

0:03

gal that fell on an outstretched hand.

0:05

She's got the SL space widening.

0:09

And I'm going to ask Dr. Stern,

0:10

could you talk a little bit about a type 2 lunate

0:12

and its relevance to SL ligament widening?

0:16

From an anatomic standpoint, there are two types of lunate.

0:17

11 00:00:20,283 --> 00:00:24,080 A type 1 lunate and a type 2 lunate.

0:22

13 00:00:24,130 --> 00:00:28,520 This happens to be a type 2 lunate, uh, in which

0:28

the hamate, uh, articulates with the lunate.

0:31

In other words, there's a separate facet

0:33

on the lunate, uh, for the hamate and then,

0:37

uh, most of the articulation,

0:39

of course, is with the head of the capitate.

0:41

And the clinical significance of this is that

0:44

this facet, uh, in long standing scapholunate

0:48

separation is protective of carpal collapse.

0:51

In other words, uh, it makes it more difficult for

0:55

the capitate to migrate approximately over time

0:58

as a wedge between the, uh, scaphoid and lunate.

1:02

What's a type 1 lunate?

1:03

Type 1 lunate is when there's no articulation

1:06

of the, uh, hamate with the lunate.

1:09

So there would be no, uh, hamate facet

1:13

right here, this small articulation with

1:15

the proximal pole of the, uh, hamate.

1:19

So is another way to say it that a

1:20

type 2 lunate is protective in some respects?

1:23

That's, that's conjecture, but

1:25

I believe that's true, yes.

1:27

Okay. Let's move on.

1:28

Dr. P and Dr. Stern out.
