They are usually readily reversible, but as in

methotrexate leukoencephalopathy, it can lead

to substantial morbidity and even mortality.

Most commonly, these are due to chemotherapy

drugs, but it can be due to illicit drugs that are

being taken on the street and immunosuppressive drugs.

be positive on DWI but FLAIR negative.

Here is an additional case of methotrexate-induced

leukoencephalopathy—DWI positive, ADC positive,

and yet nothing to be seen here on the FLAIR scan.

That's a very unusual pattern in general,

but it is a pattern that may be seen

with methotrexate or fludarabine.s
