Case: Subdural Hematoma on CT

0:00

So here we have the collection.

0:02

Remember that this is one where we're

0:04

seeing the swirl sign of active hemorrhage.

0:08

But even though it may be actively bleeding,

0:11

it is still crescentic in its appearance.

0:14

It doesn't have convex inward borders.

0:18

That would be typical of an epidural hematoma.

0:22

We also see that this collection crosses

0:25

the sutures because epidural hematomas

0:28

are between the dura and the bone.

0:30

They're confined generally by the sutures,

0:33

so the coronal suture, which we would expect

0:35

to be somewhere around here, has been

0:38

violated by this collection, and therefore, it

0:41

is more likely to be a subdural collection.

0:46

We do not see, in this example,

0:49

a fracture on the bone windows.

0:53

And the presence of a fracture of the squamosal

0:58

portion of the temporal bone would make it more

1:02

likely that you would have an epidural hematoma.

1:05

So the absence of that favors this being

1:09

a subdural hematoma in this example.

1:13

So these are the findings that would, uh, argue

1:17

strongly that this is a subdural hematoma,

1:20

rather than an epidural hematoma.
