Post-Course Activities

<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

TECHNIQUE: Multidetector-row CT of the abdomen and pelvis was performed after administration of intravenous contrast using tailored dose modulation techniques. Images were reconstructed in the axial, coronal, and sagittal planes.

COMPARISON: None.

FINDINGS:

Lower Chest: There is centrilobular and paraseptal emphysema. A large thin-walled cyst in the left lung base. No consolidation or pleural effusions.

Liver: No focal lesions.

Biliary: Status post cholecystectomy. There is mild intrahepatic biliary duct dilation, the sequela of cholecystectomy. The common duct is dilated, measuring approximately 1.5 cm.

Spleen: The spleen is top normal in size, measuring approximately 13.0 cm in craniocaudal dimension. There are no discrete splenic lesions.

Pancreas: The pancreatic duct is top normal in caliber, measuring approximately 0.4 cm at the pancreatic head, tapering toward the tail. The there are no discrete parenchymal lesions.

Adrenal Glands: Normal. No nodules.

Kidneys/Ureters: Normal. No solid masses, stones, or hydronephrosis.

Bowel: There is a small bowel obstruction with the transition point along the left pelvic sidewall. There is a small volume of mesenteric fluid. There is intraluminal visualization of the distal small bowel lobe. There is no evidence of closed loop obstruction. There is no evidence of intra-abdominal ascites. No evidence of pneumatosis or portal venous gas. The colon is normal in caliber. Status post appendectomy.

Peritoneum/Retroperitoneum: There is a small amount of pelvic free fluid. No masses or pneumoperitoneum.

Lymph Nodes: Normal. No lymphadenopathy.

Pelvic Organs/Bladder: Normal. No mass.

Vessels: No abdominal aortic aneurysm.

Bones/Soft Tissues: There is an approximately 2.2 cm lesion in the intertrochanteric left femur demonstrating rings and arcs calcifications, likely a benign enchondroma. There are degenerative changes of the lumbar spine most pronounced at L5-S1. No destructive osseous lesions.

IMPRESSION:

1. Small bowel obstruction with a transition point along the left pelvic sidewall. Fecalized small bowel and small volume of mesenteric fluid. Appearance is suggestive of a low-grade obstruction. No evidence of closed loop physiology, no evidence of internal hernia. No evidence of perforation, no evidence of drainable fluid collection.
