<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

CT ABDOMEN/PELVIS WITH CONTRAST

TECHNIQUE: Multidetector-row CT of the abdomen and pelvis was performed after administration of intravenous contrast using tailored dose modulation techniques. Images were reconstructed in the axial, coronal, and sagittal planes.

COMPARISON: None

FINDINGS:

Lower Chest: Right greater than left dependent tree-in-bud opacities. Moderate coronary calcifications.

Liver: No focal lesions.

Biliary: Normal appearance of the gallbladder. No biliary ductal dilatation.

Spleen: No splenomegaly or focal lesions.

Pancreas: No masses or ductal dilatation.

Adrenal Glands: No nodules.

Kidneys/Ureters: No solid masses, stones, or hydronephrosis.

Bowel: There are numerous colonic diverticula within the descending and sigmoid colon, with surrounding mesenteric stranding and prominent lymph nodes. There is no drainable fluid collection. The appendix appears normal.

Peritoneum/Retroperitoneum: There is stranding along the left Gerotas fascia. No masses, pneumoperitoneum, or ascites.

Lymph Nodes: Numerous prominent left para-aortic and mesenteric lymph nodes are likely reactive.

Pelvic Organs/Bladder: Prostatomegaly, with surgical clips in the prostate. There is irregular thickening of the walls of the underdistended bladder, with a small superoanterior bladder diverticulum.

Vessels: There is thrombus within the entire length of the inferior mesenteric vein to the tributaries, extending into the confluence of the portal vein and splenic vein. Calcified atherosclerotic plaque along the abdominal aorta and its branches. No abdominal aortic aneurysm.

Bones/Soft Tissues: Small bilateral fat containing inguinal hernias. Degenerative changes of the lumbar spine, with flowing osteophytes consistent with DISH, without destructive osseous lesions.

IMPRESSION:

1. Acute diverticulitis of the descending or sigmoid colon, with secondary thrombophlebitis of the inferior mesenteric vein, with the thrombus extending to the confluence of the portal vein. No drainable fluid collection or pneumoperitoneum.
2. Bibasilar tree-in-bud opacities likely reflect aspiration.
3. Prostatomegaly, with appearance of the urinary bladder likely reflecting chronic outlet obstruction.
