<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

TECHNIQUE:
CT of the abdomen and pelvis with intravenous contrast.

Scans were continued into the pelvis to evaluate abdominal pain.

COMPARISON: None available.

FINDINGS:
LOWER THORAX: There is a large right pleural effusion. There is increased opacity within the lung abutting the right heart border likely atelectasis.

HEPATOBILIARY: There is a 2.1 x 0.7 cm lesion within segment V of the liver laterally which shows some peripheral nodular enhancement which fills slightly on delayed imaging which likely represent a hemangioma. There is a 8 millimeter low attenuation lesion within segment VII of the liver which is nonspecific. In addition there is a 4 mm nonspecific low density lesion within segment IVA. There are no dilated intrahepatic ducts to the gallbladder is within normal limits.

SPLEEN: No splenomegaly.

PANCREAS: No focal masses or ductal dilatation.

ADRENALS: There is thickening of the left adrenal gland measuring about 9 mm.

KIDNEYS/URETERS: No hydronephrosis, stones, or solid mass lesions. There is a 1.4 cm cyst within the left lateral kidney and a smaller 1.2 cm cyst within the right lower kidney.

PELVIC ORGANS/BLADDER: There is calcified fibroid within the uterus. Bladder is unremarkable.

PERITONEUM / RETROPERITONEUM: There is a moderate amount of free fluid within the pelvis and in the perihepatic space.

LYMPH NODES: No lymphadenopathy.

VESSELS: There is atherosclerotic disease of the abdominal aorta.

GI TRACT: There are distended loops of small bowel throughout the abdomen. There is a loop of bowel which extends through the right obturator foramen with the transition point at this level. There is decompressed bowel seen distally. The colon is decompressed.

BONES AND SOFT TISSUES: There is generalized osteopenia. There is an old fracture of the right inferior pubic ramus. There are degenerative changes of the lumbar spine.

IMPRESSION:
1. Right obturator foramen hernia causing high grade small bowel obstruction.
2. Moderate to large right pleural effusion.
