<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

TECHNIQUE: 
CT scan of the Abdomen and Pelvis with intravenous contrast. 

COMPARISON: None available.

FINDINGS:

LOWER THORAX: There are moderate to severe coronary artery calcifications. There is retrocardiac hiatus hernia which is associated with small amount of loculated fluid

HEPATOBILIARY: Cirrhotic liver morphology. No focal hepatic lesions. No biliary ductal dilatation.

SPLEEN: No splenomegaly.

PANCREAS: No focal masses or ductal dilatation.

ADRENALS: No adrenal nodules.

KIDNEYS: No hydronephrosis, stones, or solid mass lesions. Bilateral simple renal cortical cysts measuring up to 4.3 cm and the left kidney and 4.1 cm and the right kidney. There is peripheral calcification of left lower pole renal cortical cyst. There are no renal stones or solid mass lesions identified.

PERITONEUM / RETROPERITONEUM: There is moderate volume intermediate density ascites. There is no free air.

LYMPH NODES: No lymphadenopathy.

VESSELS: Atherosclerotic disease is noted in the abdominal aorta and its major branches. There are multiple esophageal and gastric varices. There is recanalization of the umbilical vein.

GI TRACT: The stomach and proximal small bowel are dilated with air-fluid levels. The proximal small bowel measures up to 3.7 cm in diameter. There is a loop of small bowel which has herniated through an umbilical abdominal hernia, which is the transition point for the small bowel obstruction. The umbilical hernia sac measures 7.3 cm in transverse dimension. The distal loops of small bowel and the colon are mostly decompressed. There is colonic diverticulosis without evidence of diverticulitis.

PELVIC ORGANS/BLADDER: Unremarkable.

BONES AND SOFT TISSUES: Degenerative changes are noted in the visualized skeleton. No suspicious lytic or blastic lesions are identified. 

IMPRESSION: 
Small bowel obstruction secondary to incarcerated loop of small bowel within an umbilical hernia. Cirrhotic liver with moderate volume ascites with gastric and esophageal varices.
