<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

TECHNIQUE: Multidetector-row CT of the abdomen and pelvis was performed after administration of intravenous contrast using tailored dose modulation techniques. Images were reconstructed in the axial, coronal, and sagittal planes.

COMPARISON: None available.

FINDINGS:

Lower Chest: No consolidation or pleural effusions.

Liver: Hepatic steatosis. Focal fatty infiltration adjacent to the falciform ligament. No suspicious hepatic lesions.

Biliary: There is a 1.5 cm impacted stone at the neck of the cystic duct with a markedly inflamed appearance of the gallbladder which demonstrates wall thickening measuring up to 9 mm and dramatic pericholecystic inflammation. No discrete mucosal discontinuity is identified.

Ectasia of the common bile duct measuring up to 1.1 cm, minimally increased compared to prior.

Spleen: No splenomegaly or focal lesions.

Pancreas: No masses or ductal dilatation.

Adrenal Glands: No nodules.

Kidneys/Ureters: There is a 1.8 cm left inferior pole nonobstructive calculus. Numerous bilateral simple renal cysts, the largest of which is in the right inferior pole.

Bowel: No distention or wall thickening. Numerous appendicoliths within the noninflamed appendix.

Peritoneum/Retroperitoneum: There is small volume pericholecystic fluid adjacent to the second portion of duodenum. No masses or pneumoperitoneum.

Lymph Nodes: 7 mm portacaval lymph node. No significantly enlarged lymph nodes.

Pelvic Organs/Bladder: Vaginal pessary.


Vessels: Aortoiliac atherosclerotic calcification.

Bones/Soft Tissues: No destructive osseous lesions. Grade 1 anterolisthesis of L4 on L5.

IMPRESSION:

1. Acute cholecystitis.
2. 1.8 cm nonobstructive left inferior pole renal calculus.
