<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

TECHNIQUE: Limited ultrasound evaluation of the abdomen focused on the right upper quadrant. Volumetric sweeps were obtained and reviewed.

COMPARISON: None available.

FINDINGS:

LIVER: There is increased echogenicity of the liver parenchyma suggestive of hepatic steatosis. No focal lesions.

BILIARY: The common bile duct measures 7 mm. There is no intrahepatic ductal dilation.

GALLBLADDER: The gallbladder is distended with a thickened echogenic wall, associated dirty shadowing, and an obstructing stone at the gallbladder neck. This is best visualized on cine images. The structure labeled “gallbladder” on still images is favored to represent adjacent free fluid or a collection in the gallbladder fossa. There is positive sonographic Murphy sign. 

PERITONEUM: There is small volume perihepatic free fluid.

IMPRESSION:

1. Acute emphysematous cholecystitis. Perforation is suspected given adjacent fluid in the gallbladder fossa. Recommend CT for further characterization. 

2. Mildly dilated common bile duct measuring 7 mm. 

3. Hepatic steatosis.
