<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

TECHNIQUE: 

COMPARISON: None.

FINDINGS:

LOWER THORAX: Calcified granuloma at the left base is again noted. There is bilateral pleural calcifications and thickening. There is a small hiatal hernia.

HEPATOBILIARY: Subcentimeter hypodensity in segment 5/8 is too small to characterize. Cholelithiasis without evidence of cholecystitis. No focal hepatic lesions. No biliary ductal dilatation.

SPLEEN: No splenomegaly.

PANCREAS: No focal masses or ductal dilatation.

ADRENALS: No adrenal nodules.

KIDNEYS/URETERS: There is a 2.7 cm hyperdense cyst in the right interpolar region. There is a 5 mm left lower pole hyperdense cyst. There are multiple bilateral hypodense cysts measuring up to 11 cm on the right and 4.7 cm on the left. No hydronephrosis or stones.

PELVIC ORGANS/BLADDER: Unremarkable.

PERITONEUM / RETROPERITONEUM: No free air or fluid.

LYMPH NODES: No lymphadenopathy.

VESSELS: There are diffuse atherosclerotic calcifications within the aorta and its visualized branches. 

GI TRACT: There is a loop of grossly distended distal sigmoid which is twisted upon itself in the pelvis, with swirled mesentery, consistent with volvulus. There is adjacent mesenteric stranding and venous engorgement.

BONES AND SOFT TISSUES: Status post right hip total arthroplasty. Degenerative changes are noted in the visualized skeleton. No suspicious lytic or blastic lesions are identified.

IMPRESSION: 
Sigmoid volvulus.
