<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

TECHNIQUE: Multidetector-row CT of the abdomen and pelvis was performed after administration of intravenous contrast using tailored dose modulation techniques. Images were reconstructed in the axial, coronal, and sagittal planes.

COMPARISON: None.

FINDINGS:

HEPATOBILIARY: Hepatic steatosis. No focal hepatic lesions. There is inflammation of the distended gallbladder, which also demonstrates mural thickening of approximately 5 mm upstream of a gallstone impacted within the cystic duct as seen on axial images 51 and 52. There is also mass effect on the anterior abdominal wall ("tensile gallbladder fundus sign" axial image 71).

SPLEEN: No splenomegaly.
PANCREAS: No focal masses or ductal dilatation.

ADRENALS: No adrenal nodules.
KIDNEYS/URETERS: No hydronephrosis, stones, or solid mass lesions.
PELVIC ORGANS/BLADDER: Unremarkable.

PERITONEUM / RETROPERITONEUM: No free air or fluid.

ABDOMINAL LYMPH NODES: Multiple enlarged abdominal lymph nodes including 1.7 cm portacaval node, 9 mm pericholecystic node, and 8mm right lower quadrant nodes.
VESSELS: No abdominal aortic aneurysm.

GI TRACT: No distention or wall thickening. Appendicolith within the noninflamed appendix.

BONES AND SOFT TISSUES: No suspicious lytic or blastic lesions. Small periumbilical fat-containing hernia.

IMPRESSION:
1. Acute cholecystitis.
2. Right upper quadrant adenopathy is likely reactive.
3. Hepatic steatosis.
