<p>Welcome to ED Body Cases! In this course, you can tackle cases from the Emergency ED Body Mastery Course, randomized to simulate the Call experience. Refine your reading and interpretation skills by scrolling through the cases using our diagnostic web viewer. Determine your findings and answer the corresponding quiz question.</p>

TECHNIQUE: CT of the abdomen and pelvis with intravenous contrast.
COMPARISON: None available.
FINDINGS:
LOWER THORAX: There is bilateral lower lobe subsegmental atelectasis and there is a trace right pleural effusion. Heart size is normal. There is no pericardial effusion.
HEPATOBILIARY: There are changes of liver cirrhosis. The gallbladder is distended but there is no surrounding inflammation. Portal vein and superior mesenteric vein are patent.
SPLEEN: No splenomegaly.
PANCREAS: No focal masses or ductal dilatation.
ADRENALS: No adrenal nodules.
KIDNEYS: Right kidney is normal. There is a round 4 mm hypoattenuating lesion in the interpolar left kidney that is too small to characterize but likely a simple cyst. There is no hydronephrosis.
PERITONEUM / RETROPERITONEUM: There is a small amount of free fluid in the pelvis. There is moderate free air in the upper abdomen.
LYMPH NODES: No lymphadenopathy.
VESSELS: There is mild calcified atherosclerosis of the aorta and iliac arteries.
GI TRACT: The colon is distended, particularly the cecum. There is nodular cecal mucosal thickening. There is reactive thickening of the appendix. Although streak artifact limits evaluation of the pelvis, there is an abrupt caliber change of the distal sigmoid colon, which is suspicious for an obstructing mass (series 1, image 77). There is mild dilation of distal small bowel loops that appears to be reactive. No discrete small bowel obstruction with transition point is identified. 
PELVIC ORGANS/BLADDER: Limited evaluation due to photon starvation related to the bilateral hip arthroplasties.
BONES AND SOFT TISSUES: Bilateral total hip arthroplasties are in near-anatomic alignment. No suspicious bone lesion is seen.
IMPRESSION:
1. Large bowel obstruction with abrupt caliber change of the distal sigmoid colon, suspicious for an obstructing mass. Moderate free intraperitoneal air and free fluid compatible with perforation. Although a definite source has not been identified, the cecum is dilated and demonstrates nodular mucosal thickening suspicious for ischemia.
2. Liver cirrhosis.
