Our next patient is an 80-year-old
0:03
woman with a remote history of right
0:05
breast cancer treated with mastectomy.
0:09
So now she's having yearly screening
0:12
for the left side, and she was recalled
0:14
from the screening examination for
0:17
evaluation of a mass in the left breast.
0:20
So these are the screening views of the left
0:23
breast for this patient, and we can see a
0:25
small mass here in the left lateral breast and
0:29
it's in the left upper breast on the MLO view.
0:33
This was a 2D mammogram.
0:36
So, when the patient was recalled,
0:38
we started with 3D mammographic views.
0:43
So,
0:43
we have
0:44
the MLO view and tomosynthesis images.
0:51
So, as we scroll through the tomosynthesis
0:53
images, we can see this little mass in the
0:55
upper outer left breast.
1:00
And a CC view.
1:05
And these were the tomosynthesis
1:07
images of the left CC view.
1:10
You can see that mass here.
1:14
We also did spot compression
1:16
views in CC and MLO projections.
1:19
So here's the CC spot.
1:21
You can see that mass a little better.
1:25
These are the tomosynthesis
1:27
views showing the mass.
1:32
And MLO spot compression view,
1:38
again showing that small mass here.
1:42
We
1:43
thought that we could probably see this based on
1:45
its location, that we could see it on ultrasound.
1:48
So we went to ultrasound next.
1:51
And here's the little hypoechoic mass,
1:55
in the left breast at the 12:30 position,
1:57
seven centimeters from the nipple.
1:59
And it's about four millimeters,
2:04
three to four millimeters in size.
2:05
So very small, but you can see it kind of
2:08
has irregular borders and distinct margins.
2:12
And we recommended biopsy for this.
2:16
And we did an ultrasound-guided biopsy
2:18
and it was an invasive ductal carcinoma.
