So our next patient is a 33-year-old lactating
0:03
woman with a palpable lump in the left breast.
0:07
So for this patient who is 33 but
0:10
lactating, we started with ultrasound
0:13
in the area of palpable concern.
0:15
So that was in the left breast,
0:17
just a little bit lateral to the nipple.
0:19
And we saw this hypoechoic irregular
0:22
mass, sort of taller than wide.
0:27
Measuring about two centimeters
0:28
in greatest dimension.
0:30
Here it is in antiradial and radial plane,
0:37
little blood flow around the edges, not internally.
0:40
And we were pretty worried about this.
0:43
So we knew we were going to recommend a
0:45
biopsy for it, but just to complete her
0:47
workup, we recommended a bilateral mammogram.
0:51
So that was done on the same day.
0:54
So, we did CC (craniocaudal) and MLO (mediolateral oblique) views of both
0:57
breasts, and we can see that in the left
1:00
breast, slightly lateral to the nipple and
1:02
posterior depth, there's this asymmetry,
1:05
and we also see it on the MLO view.
1:10
We're not really seeing anything
1:11
obvious in the other breast, but
1:12
just to have a closer look at this.
1:16
We looked with tomosynthesis, so this is our
1:18
mass that we've just looked at with ultrasound.
1:23
You know, nothing else in this breast.
1:27
We looked on our MLO.
1:30
This is our MLO tomosynthesis view.
1:34
We actually saw that there was one other
1:38
mass that we were interested in,
1:41
and we went back to ultrasound and found a
1:43
cyst in that location, so that was okay.
1:47
We looked at the other side, CC view,
1:51
38 00:01:55,015 --> 00:01:56,145 nothing there.
1:56
And then MLO view looked good as well.
2:04
Okay.
2:04
So, you know, we've done an
2:06
ultrasound and a mammogram.
2:08
We are finding one mass that we're
2:10
concerned about, and that was
2:12
recommended for ultrasound-guided biopsy.
2:16
And then the biopsy result was invasive
2:19
ductal carcinoma, poorly differentiated.
