Impact of T2 Sequence

0:00

Well, we did a retrospective review of that,

0:03

looking at the, um, with sequential reads without

0:06

T2 and then with T2 to see how it impacted our

0:10

interpretation of these studies, abbreviated ones.

0:12

Again, T2 takes about 4 minutes versus

0:15

the, um, the pre-GAD and the post-GAD.

0:18

Make a whole study of 8.6.

0:20

9 00:00:21,150 --> 00:00:22,420 And then that reconstruction,

0:22

get her off the table.

0:24

T2* impacted the clinical decision

0:26

making only in about 5 of the 196 patients.

0:30

So 2.5%.

0:31

15 00:00:32,220 --> 00:00:33,700 And I'm going to show you examples.

0:33

Again, this was a small series of about 200 women.

0:37

And here you can see, let me see.

0:39

I highlight 2 cases.

0:41

You can see the BI-RADS category

0:43

without the T2 or actionable.

0:45

They were recommending biopsy category four.

0:48

But when the T2 was made available,

0:51

that lesion was downgraded to benign.

0:53

I'll show you examples, things like fibroadenomas,

0:56

which have a very typical appearance on MR.

0:58

But you need that T2.

1:01

And three cases, whoops, yeah, were

1:04

downgraded from category three to benign.

1:09

So T2 did help.

1:11

The question is, does it need to be every year?

1:13

If she had sequential studies and she

1:15

had a T2 last year, does she need one this year?

1:18

I can tell you we still include

1:19

our T2 in our abbreviated protocol.

1:22

Here's an example.

1:23

I hope you can see there's a small

1:25

enhancing lesion over on the right side.

1:29

There's a little blown-up image,

1:31

her T2; it's obviously T2 bright.

1:35

And so this was downgraded to benign, looks

1:37

like, you know, sort of a fibroadenoma, and it

1:40

continued; it was fine on follow-up as well.

1:43

Here's another enhancing sort of

1:45

lobulated mass in the inferior left breast.

1:48

This was going to be given a category 3.

1:50

Three.

1:50

Based on this imaging, which you

1:52

can see on the T2, all the way on

1:54

the right there, that it really has

1:56

the septation; it's very, very bright.

1:59

It looks like a fibroadenoma or maybe it's a funny

2:02

lymph node, but either way, it's a benign finding.

2:05

So the impact of T2 sequence on

2:07

the abbreviated MS, it affected

2:10

management in about 2.5 percent,

2:12

averted biopsy and 0.1 percent

2:16

six-month follow-up, and about the same

2:18

additional evaluation in one patient.

2:20

It did not affect the

2:21

interpretation in any cancer case.

2:24

And so we really need to investigate this more.
